I hereby authorize AFSCME Council 24, hereinafter called AFSCME, to initiate bi-weekly debit or charge entries in the amount(s) specified above to the bank or debit/credit card account specified. This authorization is to remain in full force and effect until I revoke or change this authorization, which I may do at any time by notifying AFSCME Council 24 (NOT your local union) in writing of such revocation or change in such time and manner that AFSCME is afforded a reasonable opportunity to act on it and at least five (5) business days before the next scheduled debit or charge entry. If I believe an erroneous debit has been made, I have the right to have the amount of the erroneous debit immediately credited to my account by Bank up to fifteen (15) days following issuance of the statement or fourty-five days after the charge, whichever occurs first. Notice of erroneous debits may be sent to AFSCME Council 24, 8033 Excelsior Drive, Suite C, Madison, WI 53717; phone number 608.836.0024. Debit or charge entries will be initiated on each pay day. A record of each transaction will appear on my account statement and will serve as my receipt. I understand that if a debit entry is returned by my Bank as NSF or Uncollected Funds that the debit entry will be automatically resubmitted to my bank account and I may be charged a return fee up to $25.00.
